Where you'll be working
Joining the NSLHD ACAT will mean you will be part of a dynamic team with various sites providing Aged Care Assessment to NSLHD. Excellent education and clinical supervision is provided on an ongoing basis. The team will welcome applications from all those with a strong interest and passion for working with older persons and who want to be part of an experienced friendly team of Occupational Therapists, Physiotherapists, Social Workers, and Registered Nurses. Applicants need to have a strong commitment to working to a high standard within very tight time frames. Based at Ryde site.

What you'll be doing

The primary purpose of this position is the assessment of the needs of frail older people and their carers in public and private hospitals, in the community and in residential aged care facility (RACF) settings and organisation of support services, as appropriate.

A key function of this assessment process is the determination of the client's eligibility for approval to access aged care services as set out by the Aged Care Act (1997).

Candidates will need to meet the following criteria:

  • Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.
  • Relevant tertiary qualifications as per clinical discipline (nursing, occupational therapy, social work, physiotherapy), with full AHPRA registration or eligibility for membership of the Australian Association of Social Work. Current NSW drivers licence.
  • Proven high standard of written and verbal communication skills. Written skills need to be precise, clinically sound and completed in a timely manner.
  • Demonstrated discipline specific clinical experience working in relevant aged care settings and proven ability to undertake comprehensive assessments, formulate, and implement appropriate care plans for older people.
  • Have detailed knowledge of My Aged Care, related aged care programs and services with proven ability to use this knowledge to assess and support both routine and complex aged care presentations.
  • Proven clinical skills in the assessment of dementia and other age related diseases.
  • Proven ability to work independently as a clinician and collaboratively as a member of multidisciplinary teams and to work flexibly across and within all ACAT sites.
  • Demonstrated efficient keyboard skills with proven ability to use Microsoft Office Programs, electronic patient record systems and e-learning sites.
