An exciting opportunity has become available for a Full Time Community RN to work as part of our home care team. The position is based in Mt Gravatt and will involve visiting home care package clients across the Greater Brisbane area as well as occasional travel to West Moreton and Toowoomba.

As an integral member of a multi-disciplinary team, you will have a strong emphasis on holistic care with a wellness focus for the frail aged and people with dementia. Utilising your exceptional clinical and people skills, you will have a caring and empathetic disposition. This role includes a varied caseload and autonomy in providing clinical nursing care as well as clinical support to the team of case managers and home care workers.

Qualifications & Skills

• Experience in the provision of clinical care and comprehensive nursing assessments as a AHPRA Div 1 Registered Nurse
• Flexibility with travel around greater Brisbane
• Demonstrated experience in community care highly regarded
• High standard of clinical nursing care - wound, incontinence, falls prevention, cognitive assessments (PAS), and medication management
• Well-developed communication skills with the ability to relate effectively to a wide range of people
• Ability to manage and prioritise a diverse caseload with minimal supervision
• Current Covid-19 Vaccinations
• National Police Clearance Certificate (upon role offer)
