Aboriginal Health Definition:

As an Aboriginal Community Controlled Organisation we accept and respect the concept, perception and definition of health from the National Aboriginal Health Strategy as being a key concept in Aboriginal health. Health to Aboriginal peoples is a matter of determining all aspects of their life, including control over their physical environment, of their dignity, of community self-esteem, and of justice. It is not merely of the provision of doctors, hospitals, medicine or the absence of disease and incapacity. Health is not just the physical wellbeing of an individual but the social, emotional, and cultural wellbeing of the whole community. This is a whole of life view and includes the cyclical concept of life-death-life.

PRIMARY OBJECTIVES AND POSITION SUMMARY

At DWECHS we are committed to our objectives of practicing sound health care within a Primary Health Care Framework, providing safe and quality care and services aligned with aged care industry requirements and regulation, administering additional community and educational support through appropriate community and educational programs, and conducting all of the above in accord with our philosophy in an Aboriginal Community Controlled Health Organisation.

We are committed to supporting and promoting the vocation and values of Dhauwurd-Wurrung. We do so by managing services provided by our staff in an efficient and effective manner through an appropriately skilled, qualified, experienced, and sufficient workforce, who possess the right attributes, attitudes, and insights, as required by DWECHS.

As a member of our workforce, the Health Promotions/TIS Worker provides support and information for our clients, consumers, Community members and staff to “tackle” their smoking issues and Inform community on better health choices and education. Our Health Promotion/TIS Worker is required to promote healthy lifestyle choices which includes providing healthy nutrition, exercise, and smoking cessation advice. The Health Promotion/TIS Program Worker also deals assists in arranging, coordinating, and providing smoking cessation information for clients and staff. As well as arrange monthly health promotion focus areas to community to attend. Consumer and patient needs are and also collaboratively identified by the DWECH medical team, community members, consumers, patients and families, staff members and consultation groups.

SKILLS, QUALIFICATIONS, KNOWLEDGE, CAPABILITIES AND EXPERIENCE:

· Understanding of and commitment to DWECHS values, mission, and philosophy, as demonstrated via the incumbent’s actions and words

· Cert III or Cert IV in health, aged care or individual support, or other employer acceptable equivalent, firsthand experience in similar role within health care, aged care, home care or community care desirable.

· Maintains current professional registrations and where relevant any practicing certificates.

· Understanding of and ability to comply with legislation relating to the service and staff, including but not limited to: OHS, AHPRA, the Aged Care Act, Aged Care Quality Standards and related Regulations, Information Privacy Act, Health Records Act 2001, Fair Work Act 2009.

· Wholistic understanding of and overall experience in connecting with the indigenous population and aligning safe and quality care and services to individual needs, with special consideration to indigenous cultural, historical and spiritual considerations where appropriate.

· Maintains specific understanding of the Aboriginal communities that the organisation serves and knowledge of other organisations within industry that are relevant to DWECHS.

· Investigates and understands community/client individual and personal needs and contributes through engagement, promotion, and education.

· Demonstrates effective and culturally appropriate interpersonal and problem solving skills in all aspects of their role, e.g. active listening, empathy in all verbal and non-verbal communications, performance of duties and interactions with clientele, staff and colleagues, respects diversity in the team and supports colleagues, ability to guide and support others one-on-one and in a team environment, as well as assist senior members of the leadership team.

· Provides clients/community members with high quality and safe culturally appropriate care, services, and referrals.

· Respects client/community member confidentiality understands boundaries and treats clients with dignity.

· Manages time effectively to assist with planning and organizing, able to adapt quickly when faced with changing priorities.

· Demonstrated ability to provide positive role-modelling

· Ability to be part of a team, builds a positive, culturally supportive team environment and facilitates the development of team members.

· Also, able to work autonomously if needed, takes responsibility for work outcomes and helps others to understand their roles and responsibilities. May need to take direction from time to time from upper management.

· Collaborates with others from different professions and work areas to share skills and knowledge.

· Maintains awareness of knowledge, understanding and skills needed to perform role and works to address skill gaps.

· Contributes to enhancement of quality practice and ensures that own work complies with accreditation/quality standards.

· Current Victorian Drivers Licence.

· Familiar and competent in the use of Windows and other computer programs.

· Be willing to undertake training to become familiar with Communicare and other reporting systems.

· Working with Children Check or the willingness to obtain within employer specified timeframe.

· National Police Check or the willingness to obtain within employer specified timeframe.

· Effective written and verbal communication skills.

· Must be willing to undertake training and professional development including annual First-Aid and CPR training.

· Up to date Vaccination in accordance with Government legislation.

· Aboriginal or Torres Strait Islander nationality and culture preferred

INHERENT REQUIREMENTS OF THE POSITION:

· Able to satisfy and uphold mandated industry, licensing, practicing and legislative requirements, relevant to role and organization. This includes but is not limited to OHS compliance, privacy, mandated employment checks, police checks, vaccinations, training, and professional registrations.

· Able to follow all lawful and reasonable instructions of the Employer including Policies and Procedures of DWECHs, Code of Conduct, Staff Handbook and any other directive, seeks advice an support with faced with conflicts of interest and other ethical dilemmas.

DUTIES - Clinical

1. To actively participate in general staff meetings and any other relevant meetings, including staff development sessions.

2. To contribute to the further development of staff working collaboratively and an openness to learning and initiative.

3. Assist appropriate staff in maintaining infection control, as well as standards of best practice and Occupation Health and Safety where appropriate, including adequate self-hygiene and sterilisation of equipment used.

4. In conjunction with the Nurse, Healthcare Workers, and other relevant DWECHS staff, carry out smoking assessments where required and promote healthy lifestyle choices and access to health information.

5. To undertake other duties as required where legally able to do so.

Non-Clinical

1. Coordinate, assist and develop health education and counselling programs which promote healthy lifestyle choices and smoking cessation pertaining to patients, consumers, clients, staff, and the wider community.

2. Provide transport to patients to and from relevant appointments where needed. Arrange transport for patients, consumers, and clients, to facilitate attendance at appointments at Dhauwurd-Wurrung and other medical or health services.

3. Consult with other health professionals to ensure patients, consumers and clients receive appropriate care and any follow-ups. Ensure patient, consumer and client accessibility to other organisations and services and advocate on behalf of patients when required.

4. In conjunction with other health services, Dhauwurd-Wurrung staff, partners of the Glenelg Aboriginal Partnership Agreement 2011-2020 and community members, develop and support health and wellbeing initiatives, working groups, studies, surveys, and workshops.

5. Continue already established relationships with Portland District Health, SEA Change Portland, VACCHO, Glenelg Youth Network, Australian Breast-Feeding Association and look to build, develop, and establish new networks where possible.

6. Ensure all data requirements are fulfilled in accordance with protocols and guidelines given.

7. Ensure thorough discharge of reporting requirements, documentation and record keeping, including timeliness and quality audits of own and related work from time to time

8. Any other requirements as agreed to in the Service Funding contracts.

9. Within a cultural context takes initiative and supports others to look for innovative ways to improve practice and service delivery of this program.

10. Generates ideas and suggestions to enhance work practices and achieve organisational objectives.

11. Understands roles and responsibilities, fulfills work requirements, and achieves program/performance targets.

  • Initiate effective Health Promotions to deliver on agreed workplans and projects focusing on innovative approaches to health promotion, disease prevention and planning
  • Proactively identifying barriers, risks, and challenges in relation to health promotion and population planning for the purpose of developing mitigation strategies – with a strong focus of indigenous health and community engagement.

Salary: $23.16 – $42.25 per hour
