Care Coordinator to provide Chronic Care Management (CCM) and/or Remote Patient Monitoring (RPM) to patients with multiple comorbidities, or those with certain chronic conditions, including but not limited to, diabetes, cardiovascular disease, and COPD. This is a remote position and involves being on the phone with patients the majority of your day. You work with the same patients from one office month after month.

This role involves gathering and evaluating clinical information and accurately documenting your patient assessments in the EMR. You develop personalized care plans for every one of your patients.

Goals

• Provide continuous, quality care to patients to improve their overall health and well-being.

• Decreasing hospitalizations, ER visits, and other inappropriate resource utilization. Keeping our patients happy and healthy at home!

Requirements

• Certified or Registered Medical Assistant, LPN or RN licensed is required.

• Passionate about patient care, possessing the ability to relate with empathy and compassion. A passion for patient teaching is a must.

• HIPAA-Safe space to do your work. Others are not to be able to hear conversations or view screen.

• Your own computer with a reliable, fast internet connection, able to have multiple windows open at once. Headphones with microphone recommended.

Job Type: Full-time

Pay: $12.00 - $24.00 per hour
