The role:

Northern Health is a dynamic health service, providing vital public healthcare to residents of Melbourne’s northern suburbs and the surrounding regional communities. Our campuses offer services to a diverse community across acute, subacute, ambulatory and community programs.

An exciting opportunity exists for a suitably qualified Occupational Therapist - Grade 2 with sound clinical skills and commitment to quality & patient centred care to join the Northern Health Home Care Packages (HCP) program. The HCP program supports older adults with complex care needs to live at home and access their local community.

The role will include the provision of Occupational Therapy assessment and intervention to identified clients within this program who are eligible for level 3 and 4 home care packages and would benefit from OT support within their package. The Occupational Therapist will work collaboratively with the client and the case manager to identify goals and support needs to form an individualised goal orientated care plan.

This position will suit an Occupational Therapist who has a passion for providing low intensity therapy to older patients to maximise independence at home. The role requires experience in complex equipment prescription and home modifications.
