About The Opportunity
This is an exciting opportunity to be part of a dynamic and energetic multidisciplinary Transitional Care Program (TCP) team within the SWSLHD. The TCP team strives to enable clients to achieve their goals and optimise their function as a continual part of their recovery process following their hospital stay.
The Transition Care team is seeking a professional, efficient and dedicated Occupational Therapists to join their residential TCP multidisciplinary team. You will be energetic and enjoy working in a dynamic environment. You will be able to provide effective and efficient Occupational Therapy assessment, intervention implementation and discharge planning of clients in a busy residential aged care setting. You will be a strong administrator and have high levels of attention to detail and professional judgment and be able to record clinical interventions in the medical record of the patient.

What You'll Be Doing
Provide a high quality occupational therapy service to clients and their families/carers to ensure the provision of optimal health outcomes for the clients of the service. This encompasses ensuring effective and efficient entry, assessment, treatment planning, treatment implementation, evaluation and separation of patients/clients in a multi-disciplinary setting. The Occupational Therapist is also responsible for undertaking designated administrative tasks necessary to support service delivery.

How to Apply
To be considered for this position, please ensure you address the selection criteria as thoroughly as possible.
- Recognised Degree in Occupational Therapy or other qualification deemed appropriate by the employer, with registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Ability to work independently and as part of a multidisciplinary team.
- Evidence of knowledge and commitment to the application of quality improvement and evidence based practice.
- Excellent written and verbal communication skills and computer literacy in a range of programs.
- Demonstrated commitment to continuing professional education.
- Demonstrated experience in aged care and/or community aged care setting.
- Current unrestricted Australian drivers licence (P2 Licence Acceptable) subject to obtaining NSW drivers licence within 3 months of appointment.
