Healthy North Coast (Limited) is a not-for-profit organisation committed to increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time. Their priority areas include integration and coordination, mental health, drug and alcohol treatment, Aboriginal and Torres Strait Islander health, population health, aged care, maternal and child health, digital health, and the health workforce. They are currently seeking a suitably experienced person for the position of Primary Health Coordinator, within their Operations and Engagement Directorate.

Reporting to the Director, Operations and Engagement, the role is responsible for building and maintaining local relationships, and acts as a key liaison point with general practice, primary healthcare stakeholders, Local Health Districts and non-government services on behalf of Healthy North Coast. You will liaise with community members and stakeholders beyond the health sector to increase the visibility of the organisations activities and services and create opportunities for partnership.

The position will be responsible for the successful implementation of a range of initiatives that drive high performing, coordinated service delivery and person-centred healthcare. The Coordinator will build and maintain productive relationships, partnerships and linkages with primary healthcare clinicians, healthcare workers, Local Health Districts, service providers and communities.

You will project manage the implementation of short, medium and long-term healthcare improvement and clinical redesign initiatives including implementation planning, qualitative and quantitative data collection, schedule and budget reporting and risk management.

Additionally, you will develop an understanding of health needs in each community by undertaking research including needs analysis, consultation and data collection to support evidence-informed planning of programs and initiatives. This role will work closely with Primary Health and Aboriginal Health Coordinators from across the region (Tweed to Port Macquarie) to implement initiatives that improve health outcomes for Aboriginal and Torres Strait Islander people and build the capacity of mainstream providers to provide culturally safe primary healthcare services.


To be considered for this amazing community focussed opportunity, you will have gained experience in a similar stakeholder relationship driven role and enjoy building lasting relationships and promoting better outcomes through improved service delivery. You may have gained your experience within the Hospital Health sector or NFP or have a background in Nursing or GP Practice Management and be seeking a broad and interesting opportunity. You will have exceptional communication skills, both written and verbal and the tenacity to promote services to a wide range of stakeholders.
