Purpose: The Registered Nurse will be the lead clinician on each shift and will provide nursing
care to meet the clinical care needs of residents at Warramunda Aged Care Facility.


Primary Objectives: The Registered Nurse will effectively provide the clinical care needs of residential

clients located within the Warramunda Aged Care Facility.


Terms of Employment: As per the agreed employment contract and standing EBA.

The probationary period for staff employed by Warramunda Village is 6 months.

A Professional Development Review will be undertaken after three months of
employment and annually thereafter.

Qualifications: Essential:

Qualified Registered Nurse with current AHPRA registration;

Annual CPR;

Computer literacy.

Desirable

Gerontology qualification;

Aged care experience;

Physical Standards • Read computer screen 70 % of the time
(Approximate):

  • Sit 70 % of the time
  • Walk / Stand 25 % of the time.
  • Bend, squat up to 5 % of the time.

Reports to: Care Services Manager – Residential Aged Care Facility

Hours of Duty: As negotiated and rostered.

Key Areas of Responsibility

Leadership & Demonstrates a positive attitude and commitment to the agreed role and
Management organisation.

  • Guides and mentors the work practices of Enrolled Nurses and Personal Care
Workers.
  • Seeks opportunities for providing positive feedback to staff when their practice
warrants comment.

Operations Accountable for system compliance, identifying own clinical or management problem
solving and decision-making abilities when they are beyond level of knowledge and
understanding the specific requirements of Gerontic Nursing.

  • Participates in Warramunda’s Continuous Improvement Program, recording
comments, complaints and operational problems.
  • Follows the policies, procedures, work instructions and forms of Warramunda.
  • Contacts the Clinical Care Coordinator or Care Services Manager when
reportable management and clinical issues arise.
  • References journals and Aged Care Industry publications to keep up to date
with best clinical nursing practices.
  • Participates in those actions which result in the achievement of the Nursing
Service objectives.
  • Contributes to the Continuous Improvement Systems of the facility.
  • Follows the service's policies, procedures and completes forms accurately and
in a timely manner.

Key Areas of Responsibility

Professional Nursing Accountable for performing all clinical practice that complies with all relevant
Practice legislative, common law, regulatory requirements and professional standards

affecting nursing practice.

  • Acts as a nursing professional, integrating those practices which reflect the
Codes of Professional conduct, practices and ethics
  • Understands and incorporates the practices which reflect the Laws, Regulations,
Standards, Principles and Guidelines relating to Nursing practice and Aged Care
and is able to recognise and act upon breaches of law relating to practice.
  • Demonstrates a positive attitude and commitment to the position.
  • Understands the specific requirements of Gerontic nursing when providing care
and service to frail aged residents.
  • Identifies own skill gaps, plans and attends training sessions, evaluates the
result of training.
  • In conjunction with Team Leader or delegate, participates in goal setting in order
to undertake professional development.
  • Meets regularly with Team Leader or delegate to participate in performance
review of personal goals and achievements.
  • Participates in coaching activities for the achievement of organisational mission,
vision and values.

Nursing Care • Acts as a Case Manager for a group of allocated residents and is responsible for
the standard of nursing care of these individual residents.

  • Ensures the resident assessments and care plans are documented and updated
as care needs change and using the technical language required to validate the
ACFI requirements.
  • Conducts regular staff training sessions and competency assessments to
ensure both Enrolled Nurses and Personal Care Workers have the knowledge
and skills to perform their roles effectively.
  • Regularly assesses the living environment to identify care, comfort, hygiene,
security, safety and risk issues.

Resident Care Planning

  • Follows the Care Planning policies and procedures and documents
assessments, problems, strategies, evaluation of strategies for allocated
residents.
  • Conducts resident assessments, using the facility forms and tools, on entry and
throughout their stay.
  • Identifies and documents strategies to address problems associated with mental
and physical health based on current best practice.
  • Actively involves the resident/family/health care team in care planning
processes.
  • Contributes to the daily monitoring and supervising of staff practices to ensure
care staff are consistent with the care plan.

  • Actively contributes to the ‘Resident of the Day’ system, reporting variances
  • Reports changes in residents’ physical and mental condition to Clinical Care
Coordinator / Medical Practitioner.

Specialised Nursing care

  • Performs nursing interventions in accordance with recognised standards of
practice.
  • Working towards improving practice competencies which relate to specialised
nursing care.

Allied Health and related services

  • Liaises with Clinical Care Coordinator when residents care needs change and
may require referral to allied health care professional for assessment and
treatment.
  • Implements recommendations from Allied Health Care professionals and
documents in resident file

Medication Management

  • Manages medication effectively and safely, following the procedures and
completing the forms and charts accurately.
  • When delegating RN to consider: the EN / PCW.
  • Is competent.
  • Able to perform activity.
  • Person is ready to accept the delegation.
  • Provides direct and indirect supervision to Enrolled Nurses and care staff as per
allocation.
  • Report’s medication errors (own or others) immediately to Clinical Care
Coordinator using Adverse Events system or if urgent per phone.

Pain Management

  • Effectively manages resident pain and keeps residents’ as free from pain as
possible.
  • Identifies the range of care issues which may link to pain assessment and pain
strategies.
  • Evaluates the effectiveness of the strategies.
  • Palliative Care.

Nutrition and Hydration

  • Communicates the changing nutritional and hydration requirements of residents
to personal carers and catering staff.
  • Ensures residents receive adequate nutrition and hydration.
  • Ensures residents receive appropriate nutritional balance, texture and volume.

Skin care

  • Ensure residents skin integrity.

  • Manages and supervisor’s equipment in the maintenance and improvement of
resident’s skin care.
  • Contributes to the skin integrity prevention program through effective
supervision and training of care staff.
  • Uses contemporary wound management practices, reports wound healing rates
to Clinical Care Coordinator.

Continence Management

  • Contributes to the continence management program through the effective
assessment and evaluation of continence strategies.
  • Ensures continence management is effective in meeting resident’s needs.

Behaviour Management

  • Demonstrates an understanding of the principles of effective behaviour
management and implements effective strategies in a timely manner.
  • Ensures behaviour management strategies are effective in meeting resident’s
needs.
  • Assesses environment to determine how this should influence behavioural
management practices and strategies.
  • Ensures behaviour management strategies are communicated to relevant staff
as per care plan.

Mobility, dexterity and rehabilitation

  • Ensures resident’s level of mobility and dexterity is optimised.
  • Acts to improve resident independence through referrals to allied health
professional as their condition and care requirements changes.

Oral and Dental Care

  • Accurately assesses resident’s dental and oral health needs, referring to Clinical
Care Coordinator /allied health professionals as necessary.
  • Ensures resident’s oral and dental health is maintained.

Sensory Loss

  • Accurately assesses resident sensory needs referring to Clinical Care
Coordinator /Allied Health professionals as necessary, implementing
recommendations as documented.
  • Ensures resident’s management sensory losses are identified and sensory
needs are managed.

Sleep

  • Contributes to the assessment of residents’ sleep problems during the day and
night hours and evaluates sleep management strategies to enable residents to
achieve natural sleep patterns.

Key Areas of Responsibility

The Clinical Care Coordinator will assist the leadership team with implementing
effective ways for ensuring the organisational culture aligns with the agreed
strategies and underpinning values.

Health and Safety in the Team Members employed by Warramunda Village are required to participate in
Workplace Health and Safety (H&S) by:

  • Adhering to Warramunda Village policies and procedures in relation to H&S
  • Taking reasonable care to ensure that your own health and safety along with
that of others is abided to as per the OHS Act 2004.
  • Reporting any injuries, hazards or illnesses where practical, immediately to
your supervisor or H&S representative.
  • Not placing others at risk by any act or omission and not wilfully or recklessly
interfering with safety equipment.
  • Attending mandatory training.
  • Demonstrates understanding of workplace Health and Safety requirements.
  • Understands emergency response procedures.

Working Relationships Internal:

  • Internal residential aged care services teams;
  • Integration with corporate services and supporting stakeholders;
  • Supporting the Executive Management Team via the Clinical Care Coordinator;
  • Supporting the Clinical Care Manager with supervision and development of the
broader RACF team / division;
  • Supporting the broader workforce with effective clinical and workplace
compliance services

External:

  • Integration and compliance support needs of Warramunda volunteers,
contractors, visitors, clinics, GPs, consultants, specialist;
  • External governing bodies and authorities when on site (eg. Worksafe / Aged
Care Commission)
  • Warramunda supporting consultants / industry bodies (eg. LASA, MOA)

Other Requirements • Unencumbered current Police Check, Working with Children’s Check and
DWES record check

  • Minimum of two Satisfactory Referee Checks
  • Adherence to the Disability Support Worker Code of Conduct, the Child Safety
Standards and all Warramunda Village policies and procedures
  • Completion of NDIS Worker Orientation Module

  • Warramunda Village uniform or approved attire is to be worn for all shifts.

Promotion of Team Members must at all times promote Warramunda Village in a positive manner
Organisation and are encouraged to support functions that raise the awareness of the

organisation in the community.

Attendance is required at Warramunda Village functions which involve the whole
staff team and client specific events.

Networking and attendance at functions outside of normal working hours will be
required from time to time.
