An exciting opportunity for a registered nurse to join our team on a full-time basis has opened up. The person will operate the Home Care package that specializes in personalized, flexible home care so that families can keep their loved ones at home. Providing care in a person’s own home means care can be tailored specifically to the client’s needs, rather than offering a blanket approach that suits the majority.

Responsibilities

• Co-ordination of client care needs, including assessment, case management and developing individualized Care Plan
• Complete ACFI documentation and comprehensive assessments
• Working with clients, families & representatives to link with local networks GP’s, allied health, hospital teams and clinical specialists to assist clients in managing their health at home
• Providing leadership and support to Care Assistants
• Liaising with clients and their significant others, and members of a multidisciplinary team to achieve positive client outcomes
• Ensuring Compliance

Role Requirements

• Minimum Diploma in nursing with current AHPRA Registration
• 2+ years experience in a similar role
• Current First Aid and CPR
• Current Police Screening Certificate
• Driver’s License

• Proven experience with managing staff
• Mandatory annual Influenza Vaccination
• Knowledge with ACFI knowledge
• Sound clinical knowledge
• Good computer & mobile device skills
• A strong understanding of what is required to support the needs of older people
• A strong understanding of what is required to support the needs of the elderly. It is also essential to have good judgment, quick decision-making, excellent communication, staff management and administrative skills.
