The Neighborhood at Madison is hiring Resident Care Associates to join our team!

Shift Details:

PRN/Part-Time

The Resident Care Associate reports directly to the Wellness Director

PURPOSE
The Resident Care Associate is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living neighborhood. This role includes providing hands on care, physical and emotional support as outlined in each resident’s Personalized Service Plan (PSP) while maintaining a safe, comfortable home like environment. The Resident Care Associate is responsible for demonstrating all elements of wellness, remaining in accordance with federal, state and local standards and regulations and Phoenix policies to promote the highest degree of quality care and services to our residents.

PRINCIPLE DUTIES AND RESPONSIBILITIES

Analysis / Reporting

  • Reports shift updates in communication log ensuring reporting of all incidents to the nurse
  • Reports all incidents and accidents according to Phoenix policy and procedures as well as state regulations
  • Reports all unsafe and hazardous conditions / equipment immediately and according to

Phoenix policy and procedures

Resident Care

  • Participates in the development of the Personalized Service Plans and updates; attends service plan review meetings with families as requested
  • Follows the hydration schedule established for the residents is maintained during the shift
  • Completes resident meal attendance sheets have been completed and any resident who has not dined in the dining room has been provided room service and / or accounted for.
  • Knows and complies with all resident rights
  • Initiates, assigns, and assist with activities, as appropriate
  • Check all assigned residents daily for cleanliness, appropriate clothing, any continence issues and change in condition
  • Uses proper sanitary procedures and universal precautions
  • Checks on assigned resident rooms daily for general cleanliness and neatness and takes appropriate action; ensures high risk rooms are double checked
  • Assist with conducting laundry rooms, common areas – including bathrooms – checks for cleanliness and any exposed hazardous chemicals; and takes appropriate action (i.e. contacting housekeeping, personally cleaning the room, securing the chemicals)
  • Completes all ADLs for assigned residents according to the resident’s Personalized Service

Plan (PSP)

  • Responds to security system and resident call bells promptly & immediately; takes appropriate action including resetting call bells

Training

  • Completes All Phoenix Academy, job specific and management training in the timeframe designated
  • Completes all training required by the state or other regulating authorities including ongoing training per regulation

Structure

  • Participates in shift crossover meetings
  • Obtains daily RCA assignment at the beginning of each shift and submit completed assigned prior to the end of shift
  • Follows attendance policy and take breaks as assigned
  • Reviews communication log regularly throughout shift
  • Trains and orient new hires

Specific Responsibilities for RCAs assigned to Memory Care

  • Invite and assist residents to activities and in life skills and other life enriching activities as indicated on the PSP
  • Incorporate the concept of “Creating Pleasant Days” and “Joining the Journey” into the resident’s daily routine
  • Integrates the individual resident’s life skills into their daily routine
  • Blends a elements of wellness into the resident’s day

Communication / Customer Service

  • Ensures residents and families are educated about residents’ rights
  • Creates and maintains a warm professional environment
  • Attends regularly scheduled meetings with department directors and associates
  • Answers phones as appropriate
  • Maintains appropriate communications to FED and other department directors

Dining Service

  • Serves meals in the dining room an works in the dining room as assigned, if applicable
  • Promotes and ensures a pleasant dining experience during all meals
  • Assist with dining room set up and clean up as assigned
  • Follows residents diets as indicated on the PSP and confidential diet board/chart
  • Provides room service as needed
  • Practices safe food handling according to Phoenix policy to prevent an outbreak of food borne illness in compliance with universal precautions

Compliance/Safety

  • Compliant with state assisted living regulations
  • Compliant with all state and federal regulations (i.e. OSHA nurse practice acts, etc.) and

Phoenix standards as it pertains to clinical care

  • Understands and practices the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks and hazardous chemicals immediately
  • Assist with monitoring all infection control programming
  • Follows established safety regulations at all times including Personal Protective Equipment

(PPE), fire extinguishers, SDS (safety Data Sheets) and Lockout Tagout procedures

  • Reviews and demonstrates fire safety, and elopement procedures and ensures that all safety regulations are followed at all times
  • Follow instructions of the designated Lead in an emergency (e.g. elopement, fire, disaster, etc.) according to the chain of authority identified in the community disaster plan
  • Ensures Oxygen tanks are stored safely, exchanges guest/residents tanks when empty and monitors to make sure liters of oxygen are at prescribed levels

Facilities Maintenance

  • Reports any malfunctioning equipment appliances, fixtures, etc.
  • Helps to keep common area in order and complete light housekeeping task as needed and or assigned including making beds, emptying trash, putting clothes away appropriately, spot sweeping and mopping
  • Ensures that no in-use equipment is left unattended
  • Wash and fold dining room linens and napkins and set tables as needed
  • Efficiently launders residents laundry

EDUCATION/EXPERIENCE/LICENSURE- CERTIFICATION

  • High School education preferred and may be required by the state
  • Previous experience working with elders or disabled individuals, preferred
  • Must be at least 18 years of age to perform the personal care aspects of the RCA position
  • Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred
  • Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times

SKILLS AND ABILITIES

  • Previous experience working with seniors and desire to serve and care for seniors

  • Demonstrate proficiency at reading and understanding a Personalized Service Plan (PSP)

  • Understands and embraces the assisted living philosophy
  • Demonstrate working knowledge of state regulations as they pertain to the job
  • Has sound working knowledge of policy and procedures

  • Reviews and understands the orientation checklist
  • Possess written and verbal skills for effective communication and a level of understanding
