About the role
A full-time social work Level 1/2 position is available in the palliative care service at Calvary Health Care Kogarah.
The position is based 4 days per week in the Inpatient Palliative Care Unit and 1 day per week on the Community Palliative Care Team.
The social workers work closely with other social workers and the multi-disciplinary team to assist patients and family members during the last months, weeks and days of a life limiting illness. The social work department at Calvary Kogarah provides services in rehabilitation, palliative and aged care. The team are experienced and benefit from regular supervision and professional development opportunities.
Key Responsibilities
providing psychosocial assessments for people with life limiting illnesses
providing interventions for patients/clients/family members including liaison and referral to appropriate services, advocacy, anticipatory grief & loss counselling for patients and family members including children, information and assistance regarding relevant financial, legal and accommodation issues
liaison with palliative care team members including providing advice on psychosocial issues and attending ward meetings including case conferences and family meetings
What you bring
Bachelor of Social Work or Qualifying Masters in Social Work with eligibility to join the AASW
Experience in social work in a palliative, oncology or aged care setting
An understanding of the NDIS and aged care systems
A desire to join a team committed to the care of others in a mission based organisation
