Six Feet Over (SFO) is a suicide prevention and suicide loss survivor support non-profit, based in Detroit, Michigan but now expanding our suicide loss program, Lemon Aid, to Kansas. Lemon Aid coordinates and operates the State of Kansas Suicide Loss Survivor Fund with the goal of providing financial support and limited social work services to Kansas families impacted by the suicide loss of a family member. Six Feet Over is looking to hire a full time remote position for a master level social worker (MSW) to coordinate the Lemon Aid program throughout the state of Kansas, who will be supported by the program administrator and a program assistant.
Job responsibilities include:
- Review application submissions and verifies Impacted Persons initial eligibility for Lemon Aid Programing
- Develop comprehensive Impacted Persons care plans, utilizing both formal and informal support services to meet the individual and family’s identified needs emphasizing the person-centered approach to establishing a care plan that documents the issues, concerns, conditions and specific supports and interventions needed.
- Perform comprehensive evaluation including assessment of Impacted Persons unique preferences, social, emotional, physical environment, natural supports, bereavement assessments and assists in the development implementation of resource and care plan.
- Conducts ongoing reassessments of impacted persons and family needs
- Assumes active role as advocate for impacted persons and family
- Provide emotional support via brief interventions with impacted persons
- Provide short term crisis intervention when indicated
- Oversee and confirm satisfactory completion of biohazard remediation and other professional services
- Arranges needed services; monitors provider delivery of services; documents care plan outcomes
- Establish partnerships within the Kansas government formed coalitions and committees
- Identify resources through programing and community involvement
- Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
- Establish connections to collaborate with services responsive to complicated loss and/or suicide bereavement
- Coordinate with Program Administrator and Program Assistant regarding financial assistance, emotional support needs and documentation
- Assist Impacted Persons arrange funerals and memorial services
- May include working directly with service providers
- Facilitate Peer-to-Peer support
- Complete intake assessment to identify areas of need and coordinate appropriate referral
- Conduct follow up assessments with impacted persons quarterly
- Maintain documentation within casework platform
- Determine financial need of impacted persons and make appropriate recommendations to the Lemon Aid Voting Committee (LAVC)
Hours - Monday, Wednesday, Friday 9am-5pm, Tuesday and Thursday 11am-7pm
Benefit Details - Unlimited PTO (with guidelines), continued education assistance, health insurance, budgeted supervision costs up to $ 4,800 per year.
Qualifications
- Masters in Social Work (MSW) Required
- Fully licensed in the state of Kansas and under supervision of LSCSW, per licensing requirements in the state of Kansas.
- Experience working with diverse populations.
- Experience and comfortability discussing suicide loss and issues relating to death, dying and bereavement, with some crisis intervention training preferred.

Pay: $61,000.00 per year
