Mercy Community undertakes significant work assisting people in the mental health, disability, corrections and child safety space. The NDIS is one of the programs in this space where we provide assistance to people with a broad range of physical, intellectual and psychosocial impairments. People who are living with more complex impairments may require additional assistance to ensure they are accessing their NDIS funding in the best possible way -and this is where a Support Coordinator comes in!
A Support Coordinator is responsible for working directly with a person who is receiving NDIS funding, to assist them with navigating their NDIS plan, help connect them to a variety of services and relationships and check in to see how it is all coming along to ensure that the person's goals and aspirations are being met.
We currently have an opportunity for a full-time, permanent Support Coordinator in our Nudgee (North Brisbane) team. This position is at the SCHADS Award Level 4, which commences at $77,578 + Super + Salary Sacrifice
As a Support Coordinator your day may include:
- Supporting people, their families and their formal/informal support network to understand their NDIS plan;
- Completing and engaging in regular communication/reports with people, their families and NDIA;
- Identifying the most appropriate services within the community to suit an individual's needs;
- Support people to communicate their goals and aspirations to their stakeholder team and the NDIA;
- Attend home and community visits;
- Undertake general case management of a person's portfolio; and
- Manage and monitor individual NDIS Plan budgets.

What do I need for successful application?
- Ideally, you will be working towards or have completed a tertiary degree in Social Work, Psychology, Human Services, or a related discipline; however equivalent experience is also highly advantageous.
- Have experience in case management and/or care coordination.
- Highly developed adaptable and person-centred communication skills;
- Interested in building connections with disability, mainstream and community networks
- Positive and friendly personality, who is open, flexible and willing to be part of an innovative and supportive team who work together.
- An open Queensland Driver’s License;
- Yellow Card Exemption/NDIS Workers Screening Clearance and a Blue Card (or ability to obtain prior to commencement - we will pay for these if you don't have them yet!)
- NDIS Worker Orientation Certificate (this is free to do online)
- Have a minimum of 2 COVID-19 vaccinations.
